ESSENTIALS OF THE ROOT NUTRITION CONSULTING, PLLC does not accept or bill insurance for nutrition services and full payment is due at the time of your appointment via cash, check or credit card. A receipt may be obtained at your request and may also be submitted to your insurance company. Please be advised, submitting this receipt to your insurance company may not result in reimbursement for fees paid for nutrition services. 

Failure to cancel an appointment within 48 hours prior to your appointment will result in a $50 cancellation fee. All cancellations may be made via phone at (845) 803-0226.
